 Geriatric Nurse Practitioner

This Geriatric Nurse Practitioner (NP) role works in collaboration and consultation with a physician and other medical staff, nursing and members of the inter-professional health care team to respond to the needs of patients in the Hospital and the residents at St. Joseph's Manor, Long Term Care (LTC) home.

The goal of the Geriatric NP is to allow patients/residents to maximize their function and quality of life. The Geriatric NP, having a specialty-based focus, functions in collaboration with physicians to formulate clinical decisions and work with physicians to appropriately manage acute/chronic illness and promote wellness. For the LTC home function as a consultant for the BSO program area and for acute resident issues in order to support the reduction of transfers to the ED. 

 

The NP will: conduct comprehensive geriatric assessments; consult and provide recommendations to physicians on treatment (including diagnostic, pharmacological and non-pharmacological therapies and counseling); develop, implement, monitor and document a plan of care in collaboration with the physicians and interdisciplinary team; complete medication reviews for patients; provide education; refer to/liaise with other community services; and, provide follow-up as appropriate. In addition, the NP may be involved in other clinical, research, evaluation, leadership and collaborative activities (e.g. developing new standards of care, order sets, policies, etc.).

          REQUIRED SKILLS:

  • Ability to conduct comprehensive geriatric assessments and annual health exams(LTC)

  • Experience in the development, implementation, monitoring and documenting a plan of 
    care in collaboration with the physicians and interdisciplinary team

  • Ability to lead committees, and develop and implement quality improvement plans

  • Ability to conduct other clinical, research, evaluation, leadership and collaborative activities
    which can include developing new standards of care, order sets, policies, etc.)

     

    DEMONSTRATED ABILITIES:

  • Knowledge of, and willingness to support, the Values of St. Joseph’s General Hospital Elliot Lake

  • Strong leadership competencies and comprehensive understanding of patient safety issues

  • Extensive knowledge and application of College of Nurses Standards of Practice and legislation

     

    REQUIRED EXPERIENCE:

  • Current College of Nurses of Ontario registration in the Extended Class

  • Minimum 1-3 years working as a nurse practitioner (MN-NP)

  • 3-5 years of relevant Acute Medicine or Primary Care experience

  • Current certification in BCLS

  • Post-graduate education and experience in gerontology

  • Proficient in I.V. Therapy

 QUALIFICATIONS INCLUDE (but are not limited to):

  • Current Registration with the College of Nurses of Ontario

  • Current Masters in Nursing/Nurse Practitioner(MN/NP) certification

  • Geriatric competence and qualifications
