The Ministry of Health and Long Term Care / Ontario Health requires a standardized assessment to ensure treatment services are an appropriate fit; the Gain Q3-MI ON assessment should be completed to confirm our treatment model is a good match for the applicant.
To obtain an assessment in your area please contact ConnexOntario by phone at 1-866-531-2600 (24/7) or https://www.connexontario.ca
All patients seeking treatment should have a standardized assessment completed. The completed assessment summary is sent to the Camillus Department and the Intake Reception completes a short telephone screening and schedules an appointment by telephone to speak with the Intake Worker. The intake worker obtains more detailed information about the patient and provided there are no concerns, the Intake Worker will offer the earliest available treatment date.
The Camillus Intake Secretary can be contacted at (705) 848-2129 ext 3313 from Monday to Friday from 8:00 am to 12:00 pm and from 1:00 pm to 4:00 pm
